BCBA Case Manager Bundle (2026)
The documentation system for ABA case managers: authorizations, supervision, assessment, notes and discharge, in five files and more than 480 pages.
Five files, more than 480 pages
- Case Manager Field Manual, 72 pages: the lifecycle from intake and agreements through authorization, supervision, assessment, notes and discharge, with the cadence and the code map
- Authorization Report Kit, 97 pages: initial packet, goal worksheets, reauthorization, denial response, barrier analysis and medical necessity language, with payer-specific items labeled by payer because a Cigna rule is not a TRICARE rule
- Session Notes and Data Forms, 107 pages: note templates for 97151 through 97158 built to the eight documented elements, plus rate, duration, IOA, fidelity, trial, task analysis, interval and ABC sheets, and a note self-audit
- Supervision Documentation Pack, 92 pages: the RBT and BCaBA requirement grids side by side, monthly logs, BST and feedback forms, a supervision contract, competency checks and an audit self-check. Prove the five percent, the two contacts and the observation, for seven years
- Caregiver Training and Discharge, 113 pages: a twelve session caregiver curriculum you can actually bill under 97156, participation logs, fidelity checks, transition and discontinuation planning, and a discharge summary that closes clean
Every form is built against a source: the RBT Handbook and BCaBA Handbook as updated in June 2026, the CPT descriptors, published payer documentation lists, and the OIG findings above. Where a rule differs by payer or by credential, the difference is on the page rather than left for you to discover during a review.
Who it is for
This is not exam prep. It assumes you passed and now have a caseload, a supervisor role, and a payer who wants evidence.
Why documentation
In February 2026 the HHS Office of Inspector General published report A-09-24-02004 on Colorado's Medicaid payments for applied behavior analysis. Of the 100 enrollee-months sampled, 100 contained at least one improper or potentially improper claim line. Not most. All of them. OIG put the estimated improper payments at 77.8 million dollars.
Almost none of that was bad clinical work. It was documentation.
You already know how to teach a mand. What eats your week is everything wrapped around it: the packet that came back for more information, the note a biller flagged, the RBT whose supervision percentage is short with four days left in the month. This is the operating system for that part of the job.
