51 pages of WPATH SOC-8 letter templates for gender-affirming care: seven letters including hormone therapy and surgery, an assessment interview, informed consent templates and an insurance letter.

What is inside

  • 51 pages as a fillable PDF and a fully editable Word version.
  • Seven letter templates: hormone therapy, top surgery, bottom surgery, facial feminization and voice, name and gender marker change for a court petition, and insurance pre-authorization. Each one comes with a completed worked example.
  • A SOC-7 to SOC-8 comparison table covering letter counts, real-life experience, the decisional framework, non-binary guidance and adolescent care.
  • A 60 to 90 minute assessment interview structure in eight sections, with probe language for each dimension of informed consent: risks, benefits, alternatives, irreversibility and limitations.
  • Informed consent templates for feminizing and masculinizing hormone therapy, including what hormones do not change and which effects are permanent.
  • The insurance letter, which is the most formal of the set. It maps DSM-5-TR Gender Dysphoria criteria, states medical necessity, cites SOC-8 and the Endocrine Society guideline, and carries the ICD-10 and CPT framework.
  • A letter audit checklist to run before anything goes out: correct name throughout, correct pronouns, SOC-8 cited, capacity documented, recommendation stated plainly.
  • What to do when callouts at the end of every chapter, for the awkward cases. The surgeon who still wants two letters. The client with active severe symptoms. The client who has not pursued social transition.

Who it is for

  • Clinicians writing gender-affirming letters: LCSW, LMFT, LPC, PsyD, PhD and psychiatrists. Useful whether you are updating templates from SOC-7 or standardizing letters across a group practice. It assumes you have already completed training in gender-affirming care. This is a clinical operations reference, not training, and not legal advice.
  • Worth reading first: WPATH SOC-8 letters: what changed and what to include

Why it exists

A client sits down at intake. She has read WPATH SOC-8, she has been in social transition for three years, and she wants a letter so her doctor can start estrogen. Her insurer wants pre-authorization paperwork that cites the medical policy. Her surgical consult in three months wants a separate letter for top surgery. The intake template that works for an anxiety case will not produce any of those, and a letter carrying her old name comes back rejected.

Most letter templates circulating online still reflect SOC-7. This kit is built on SOC-8, which changed the requirements in ways that matter: one letter instead of two for most procedures, no real-life experience requirement, and an informed consent frame rather than an eligibility check.

Why SOC-8 changed the letter

Under SOC-7 the letter was an eligibility determination. Under SOC-8 it documents that the person has capacity and has given informed consent. That is a different document with different language, and a letter still written in the older frame reads as out of step to anyone who knows the standard.

SOC-8 also states that a mental health diagnosis does not preclude gender-affirming care. The question is capacity. The templates carry that framing explicitly, so a diagnosis on file does not get turned into a barrier by careless wording.
