Who it is for
Licensed and supervised clinicians who assess and document suicide risk.
Why it exists
Risk work has two jobs that happen at the same time: help keep the person safe, and leave a record showing that a careful clinician made reasonable decisions on the information available. Training covers the first job. This system is the structure for both.
The guide walks the assessment frame domain by domain, ideation, plan and intent and means, history, drivers, protective factors, and change, toward a formulation rather than a score. It covers the level of care decision and the two disciplines that improve it and the record at once: consultation, documented, and the road not taken, written down. Then safety planning that has evidence behind it, the six part collaborative plan in the client's own words, and the means safety conversation made concrete, in place of the no-suicide contract the literature never supported. The documentation chapter is a table of weak note language against stronger, with the reasoning: what you assessed, you can defend.
Format
Six page PDF guide plus editable Word forms, with a printable PDF set of the same.
A process and documentation resource for licensed and supervised clinicians. It is not clinical training in suicide risk assessment, does not establish a standard of care, and is not legal advice; formal training, supervision, consultation, statutory duties, and agency policy govern.
There is a free video lesson on documenting suicide risk, with a walkthrough of what is inside the file.
