School FBA and BIP Kit (2026)
A complete functional behavior assessment and behavior intervention plan kit for school teams. 434 pages across five printable files: the assessment forms, the report template with two complete worked examples, the plan templates with 124 written strategies and 48 IEP behavior goals, and the discipline and consent paperwork that surrounds all of it.
What you get
- The FBA and BIP Field Manual, 70 pages. What the law actually requires, with the citation next to every claim, and a table separating what is required from what is conditional from what people simply assert in meetings. Parental consent for an FBA, stated the way the November 2024 federal guidance states it. The ten school day rule, change of placement, manifestation determination, the forty five school day special circumstances, and informal removals. Then the technical layer: the functions of behavior done correctly, indirect and descriptive assessment and their real limits, functional analysis, measurement systems and their bias, interobserver agreement, and what makes a plan technically adequate.
- FBA Data Collection Forms, 96 pages. Original staff, caregiver and student interviews. Removal day tracker. Operational definition worksheet. Setting event, antecedent and consequence inventories. ABC narrative and structured recording, scatterplot, frequency and rate, duration, latency, interresponse time, partial interval, whole interval, momentary time sampling, permanent product, a conditional probability worksheet, trial based functional analysis, agreement sheets, preference assessment and a formula quick reference. Every formula is printed on the sheet that needs it.
- FBA Report Template and Two Worked Examples, 84 pages. The full report template section by section, then two complete fictional reports written the way a report should be written: real numbers, real observation counts, real agreement figures, a conditional probability calculation shown with the arithmetic, and honest limitations. The second example is deliberately harder, with indirect and descriptive data that disagree and a hypothesis the team could only state tentatively. Plus a self review checklist and a page of language to avoid in a report, with the replacement.
- BIP Templates and Strategy Bank, 91 pages. The plan template built to the technical adequacy criteria, plus the strategies already written: 35 setting event strategies, 74 antecedent strategies organized by function, functional communication training planning, differential reinforcement procedures, consequence modification, crisis procedures, progress monitoring and fidelity plans, a one page plan for the classroom, and a bank of 48 measurable IEP behavior goals with 16 objectives.
- Discipline, Consent and Team Compliance Pack, 93 pages. The discipline decision tree with a citation at every branch. Removal tracker, pattern analysis worksheet, informal removal log, manifestation determination review record, special circumstances documentation, two FBA consent forms with a decision aid for choosing between them, meeting agendas and minutes, training rosters, fidelity checklists, three parent letters and an annual file audit.

Why it is different
Two things sink a school behavior case. The first is a legal error: a team asserts a requirement that does not exist, or misses one that does. The second is a technical adequacy error: the hypothesis rests on no data, or the plan reinforces a replacement behavior that produces a different outcome than the problem behavior did. This kit is built around both.
Every legal statement carries its regulation number, and the kit is explicit about the difference between what IDEA requires and what is best practice. It states the federal floor and says plainly that state law and district policy add to it. No copyrighted assessment instrument is reproduced anywhere: the named instruments are cited and you are told where to obtain them.

This is a professional practice resource, not legal advice. It states the federal requirements under IDEA. Your state, your district and your own counsel add to them. Verify against your local requirements before you rely on it.
