NCMHCE FAQ 2026: Case Format, Time and Passing Score

The NCMHCE is 11 case studies. Ten of them are scored.

What is the format of the NCMHCE?

The National Clinical Mental Health Counseling Examination is built from 11 narrative case studies rather than a bank of standalone questions. Each case carries 9 to 15 multiple choice questions with four options, for a total of 130 to 150 questions on a given form. One of the 11 cases is unscored and is being trialed for future forms. It is not marked, so you work every case as though it counts. 100 questions are scored.

How long do you get for the NCMHCE?

255 minutes, which is four hours and fifteen minutes, for the whole exam. Because the questions come in case sets rather than one at a time, pacing is best planned per case: eleven cases in 255 minutes is a little over 23 minutes each.

How is the NCMHCE weighted?

NBCC publishes six domains. Counseling Skills and Interventions is 30 percent. Intake, Assessment, and Diagnosis is 25 percent. Professional Practice and Ethics is 15 percent. Treatment Planning is 15 percent. Core Counseling Attributes is 15 percent. The sixth domain, Areas of Clinical Focus, is listed at zero percent with a footnote, and that footnote matters.

Why is Areas of Clinical Focus listed at zero percent?

Because it is not tested at the item level. NBCC's footnote says the domain represents the diagnoses and presenting problems the job analysis found most prevalent in clinical work, and that it is evaluated through the variety of diagnoses and scenarios appearing on each form rather than through dedicated questions. The other five weights add to 100 percent, which confirms it. Some prep material reports this domain as 1 percent, which is a text extraction error reading the footnote marker as a digit. It is zero.

What is a passing score on the NCMHCE?

NBCC does not publish a cut score for the current form. Results are pass or fail, set through NBCC's standard setting process, and no fixed number of correct answers guarantees a pass.

Is the NCMHCE changing?

Yes. NBCC has published 2027 exam specifications effective July 1, 2027. The new form has ten cases rather than eleven, 30 to 50 field test items scattered across the cases rather than one wholly unscored case, 225 minutes rather than 255, a published scaled passing score of 360 on a 100 to 500 range, and six renamed domains weighted 15, 18, 15, 20, 12 and 20 percent. It was built from a 2024 to 2025 work analysis. If you test before July 2027, the current outline applies. If after, study from the 2027 specifications.

What is the difference between the NCE and the NCMHCE?

Both are NBCC exams. The NCE is a 200 question multiple choice exam of general counseling knowledge. The NCMHCE is the clinical exam, built from case studies, and it tests clinical decision making rather than recall. Which one your state requires, and at what stage of licensure, is a question for your board, because states differ and some require both.

What should you study first for the NCMHCE?

Counseling Skills and Interventions, at 30 percent, is the single largest domain and the one a study plan should be built around. It is also where most competing practice sets are thin, because writing case embedded skills questions is harder than writing diagnosis questions. Intake, Assessment, and Diagnosis at 25 percent comes second. Between them those two domains are more than half of the scored questions.

Our Clinical Mental Health Counseling Exam Prep bundle is built to NBCC's current outline, with Counseling Skills and Interventions weighted at the 30 percent the exam actually gives it.

Score yourself by section

The free NCMHCE mini exam is 40 questions in the case based style the exam uses, weighted to the five domains, with a written rationale for every answer. Score it by domain rather than overall, then put your next study hours into the domain furthest behind its weight. No card and no code.
